Checklist: What to Bring to Your Consultation?

To ensure a smooth registration process, it's best to bring a few things with you to your appointment. We'll list them for you.

Always bring your eID (electronic identity card), KidsID, or ISI+ card to UZA.

This is important because it is the only way for us to apply the third-party payer system. With this system, you only pay your share of the costs to the healthcare provider (co-payment), and other costs are reimbursed directly by your health insurance.

Starting from 01/01/2024, if you don't have your eID, KidsID, or ISI+ card with you at the hospital, you will always have to pay a deposit for medical services. Without reading your eID, ISI+ card, or KidsID, the regulations no longer allow for the third-party payer system.

Checklist

For your registration:

  • Electronic identity card (eID), KidsID, ISI+ card
  • Telephone number or mobile number
  • Name and address of your general practitioner
  • Name and address of the referring physician
     

In the following cases, you will need additional information:

  • For a work-related accident:
    - Name and address of the employer
    - Name and address of the insurance company
    - Date of the accident
    - Claim number (if known)
    - Policy number
  • For an expertise examination, you will also need the referral letter from the instructing party.
  • If applicable to you, bring a guarantee arrangement from a Public Centre for Social Welfare (OCMW) or a payment commitment from another institution guaranteeing the payment of costs.
  • For patients with a foreign residence (domicile):
    - Insurance card or E112 form or guarantee declaration from your health insurer
    - European Health Insurance Card

For the medical team:

  • List of medications you are taking
  • Indication of any allergies
  • Medical documents you have: referral letter from your general practitioner or referring physician, radiographs, laboratory results, vaccination card, and blood type card.
